Role Description

We are looking for a full-time Home Health Care Coordinator to join our team in Edmonton, AB. This is an on-site role. The Home Health Care Coordinator will be responsible for coordinating and managing client care plans, scheduling home health care services and staff, communicating with clients and their families, and ensuring compliance with care standards and regulations. The role involves daily interactions with the health care team to ensure the delivery of high-quality care.

  • Experience in Dementia care is a plus
