Right at Home Ottawa is looking for a PSW Team Lead / Junior Care Planner to support our growing team of caregivers and clients across the community.
This role is perfect for someone who enjoys a balance of client interaction, care coordination, and team mentorship while contributing to exceptional service quality and client satisfaction.
Role Description
In this full-time, hybrid role based in Ottawa, the PSW Team Lead / Junior Care Planner supports both clients and caregivers to ensure high-quality, compassionate home care. Key responsibilities include:
- Coordinating day-to-day care delivery by working closely with the Scheduler and Care Planner.
- Conducting client assessments, developing personalized care plans, and updating them as needs evolve.
- Supervising and mentoring PSWs, providing ongoing feedback and coaching to uphold care standards.
- Acting as a primary contact for clients and families, ensuring effective communication and timely support.
- Participating in recruitment, onboarding, and training of new caregivers.
- Maintaining accurate documentation and reporting in the tools used.
Qualifications
- Certification in one of the following: Personal Support Worker (PSW), Social Service Worker, Occupational Therapist Assistant / Physiotherapist Assistant, or recent RPN graduate (eligible to practice in Ontario).
- Post-secondary education in a related field (e.g., healthcare, social services, business, or administration) is preferred.
- 2–3 years of experience in home care, community health, or administrative coordination.
- Strong leadership and interpersonal skills with the ability to mentor and support caregivers.
- Proficient in Microsoft Office Suite (Word, Excel, Outlook) and comfortable using client databases and scheduling systems.
- Excellent written and verbal communication, organizational, and time management abilities.
- Current First Aid and CPR (Level C) certification and Vulnerable Sector Check (issued within 6 months).
- Valid driver’s license, reliable vehicle, and proof of insurance required.
