Position: Wellness Coordinator - Addictions
Department: Health & Community Wellness
Employment Status: Full-time
Salary: $31.34 37.46 /HR 35 Hours per Week
Location: Naughton, ON
Closing Date: September 24, 2025, at 4pm
POSITION SCOPE:
The Wellness Coordinator facilitates topics of mental health and addictions for the communitys education and awareness that allows people in the community to see how to remove barriers and choose a healthy lifestyle.
REQUIRED QUALIFICATIONS:
- Bachelor of Arts Degree in Social Work, or,
- College Diploma in Social/Health Science, or equivalent.
- 5 years work experience in social/health settings
- CPIC (Vulnerable Sector)
- Valid Ontario Class G Drivers License with a clear drivers abstract
KEY RESPONSIBILITIES:
- Provide assistance to individual(s)/families through the case management model
- Provide intakes and assessments and develop plans of care for clients
- Provide one on one support
- Develop and facilitate therapeutic support groups
- Complete referrals as necessary
- Develop health and wellness programming for the community
- Establish educational goals in the form of an annual learning plan
- Prepare monthly calendars and participate in program planning meetings
