We are seeking a dedicated Family Physician to join our established practice in Toronto's vibrant Dufferin and St. Clair neighborhood. The Opportunity

Our practice operates under the Comprehensive Care Model (CCM) with plans to transition to a Family Health Group (FHG) model as we expand to three family physicians, with long-term goals of establishing a Family Health Organization (FHO). We are looking for a Family Physician who:

• Holds current licensure or eligible for a license with the College of Physicians and Surgeons of Ontario


• Values both independent practice and collaborative healthcare delivery


• Is committed to comprehensive family medicine across all age groups
